If you plan on making a change to your child's transportation, please choose from the two following procedures:
Temporary Changes
Please use this form to change transportation temporarily. Fill out the online form and then email a copy of your photo ID to 630.masonrequests@gcpsk12.org to make the request official. If you do not email your photo ID and we cannot reach you by the phone number on file, your request cannot be fulfilled. Only a parent may request a temporary change in transportation.

Permanent Changes
If you need to make a permanent change to your child's mode of transportation, you need to download this form, fill it out, and bring it personally (with a photo ID) to the Mason Elementary Front office. This is a GCPS district policy. Only a parent may request this change.
